At the beginning of the project, we felt disconnected to each other and the community.  We had weak ties for transitions to and from our local hospital and other community services.  We wanted to enhance our relationship with other campus services.

As we expanded the mental health services available, a care plan was developed to communicate with other care providers about the services offered and appropriate diagnoses for those services. Our mental health nurse (MHN) and project manager, under the guidance of our director, began building a pathway and enhancing relationships.  As we built programs, the following treatment pathway emerged to help students navigate entry into the system.  Notice that treatment pathways inside our system are multi-directional.
